RN MDS Coordinator

RN- Minimum Data Set Coordinator
Holly Heights Nursing Center, a premier 5-star facility is currently seeking for a qualified RN MDS Coordinator for our LTC Facility.

Qualified candidates will process the following:
– Active Colorado RN License
– Excellent customer service
– Excellent clinical skills

– The ability to multi-task

Position Details:
-Assist in developing personalized Resident care plans, identify and anticipate Resident needs as required by both Federal and State Regulations.
-Actively contribute to the Nursing Center interdisciplinary Resident Assessment Instrument (RAI) process while adhering to RAI guidelines for completion, computer encoding and electronic submission of MDS.
-Be proactive in identifying changes in patient condition in conjunction with the interdisciplinary team and coordinate scheduling and completion of indicated assessments.
-Assist with Activities of Daily Living training for new employees and through continuing education to existing staff members.
-Attend Educational opportunities to maintain knowledge and stay current with RAI/ Prospective Payment System regulations.
