Full Time Volunteer Coordinator

The Little Sisters of the Poor – Mullen Home is a not-for-profit nursing and assisted living facility in the heart of the Highlands District in Denver, Colorado. The property sits on 11 acres with beautiful landscaped gardens. Since 1917, the religious order of Little Sisters of the Poor have been active at the Mullen Home in service of the needy elderly in the Denver area.

JOB DESCRIPTION
The Volunteer Coordinator serves as a role model in upholding the philosophy, objectives, and goals of the Little Sisters of the Poor.
- Acts as a regular (daily) back up for the Receptionist by operating a multi-line telephone system and following appropriate procedures at the front desk. -Will plan, develop, implement and direct the volunteer program in a manner that successfully meets the needs of the Residents and our Home.

- Will assure that each volunteer receives appropriate instruction and training as may be required by City, State or Federal regulations to carry out assignments effectively and safely.
- Will offer consistent and individualized attention and affirmation that will foster volunteer retention. Will write volunteer evaluations.
- As a team member, will work effectively and cooperatively with the Home’s administration and all departments.
- Assesses on an ongoing basis, but at least annually, the effectiveness of the volunteer program and revise accordingly.
POSITION REQUIREMENTS
- Experience as a volunteer coordinator and/or in a non-profit setting preferred.
- High school education; minimum.
- Knowledge of community and volunteer resources; ability to find and utilize such resources.

Position Summary:
Under limited supervision, plans, directs, and evaluates total programming and ementia care within the context and according to the guidelines set forth by the Legacy Courts program. Helps staff understand that emotional and social displays of care positively affects the residents. BASIC FUNCTIONS
The primary responsibility of your job position is to assist the Dietician in planning, organizing, developing and directing the overall operation of the Food Service Department following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are entrusted to assure that quality nutritional services are provided on a daily basis and that the Food Service Department is maintained in a clean, safe, and sanitary manner.
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Coordinate food services and activities with other related departments.
- Assist in developing and maintaining written food services policies and procedures.
- Interpret the department’s policies and procedures to residents, employees, visitors, government agencies, etc., as necessary.
- Perform administrative duties such as completing necessary forms, reports, evaluations, studies, etc to assure control of equipment and supplies.
- Develop and maintain a file of tested standard recipes.
- Inspect food storage rooms, utility/janitorial closets, etc., for upkeep and supply control.
- Interview residents or family members, as necessary to obtain diet history.
- Participate in maintaining records of the resident’s food likes and dislikes.
- Provide substitute foods or similar nutritive value to residents who refuse food served.
- Assist in developing methods for determining the quality and quantity of food served.
- Ensure that menus are maintained and filed.
- Assist in planning regular and special diet menus to assure they are in compliance with the physician’s orders.
- Evaluate and implement recommendations from established committees as they may pertain to food services.
- Assist in the recruitment, interviewing, and selection of food services personnel.
- Schedule department work hours.
- Counsel/discipline food service personnel as requested of as necessary.
- Terminate employment when necessary, documenting and coordinating such actions with the HR Director and/or Administrator.
- Monitor absenteeism.
- Assist in establishing a food service production line.
- Conduct departmental performance evaluations.
- Assist in interviewing/obtaining food supply vendors.
- Assist in the purchasing of food service supplies, equipment, etc., as required.
- Assist in planning department’s budget.
- Maintain confidentiality of all pertinent resident care information including protected health information.
- Demonstrate a passion for caring as evidenced by interaction with co-workers, residents, families, and visitors. • Perform all other duties, as assigned.
EDUCATION/EXPERIENCE/JOB TRAINING
- Must possess, as a minimum, a high school diploma.
- Be a graduate of an accredited course in dietetic training approved by the American Dietetic Association. Experience
- Must have, as a minimum five (5) years experience in a supervisory capacity in a hospital, nursing care facility, or other related medical facility.
- Must have training in cost control, food management, diet therapy, etc.
Specific Requirements
- Must be able to read, write, speak, and understand the English language.
- Must be registered as a Food Service Director in this state.
- Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
- Must be knowledgeable of food services practices and procedures as well as the laws, regulations, and guidelines governing food services functions in nursing care facilities.
- Must maintain the care and use of supplies, equipment, and maintain the appearance of food service areas.
- Must perform regular inspections of food services service area for sanitation, order, safety, and proper performance of assigned duties.
- Must have the ability to plan, organize, develop, implement, and interpret the programs goals, objectives, policies, procedures, etc ., of the Food Service Department.

BASIC FUNCTIONS
The primary responsibility of your job position is to plan, organize, develop, and direct overall operation of our Nursing Service Department following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are entrusted to ensure that the highest degree of quality care can be provided to our residents at all times.
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Plan, develop, organize, implement, evaluate, and direct the nursing service department, as well as its programs and activities.
- Develop, maintain, and periodically update written policies and procedures that govern the day-to-day functions of the nursing service department.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
- Determine the staffing needs.
- Recommend the number and level of nursing personnel to be employed.
- Assist in calculating the number of direct nursing personnel care on duty each shift.
- Ensure that all nursing assistants are enrolled in or have graduated from an approved training program.
- Develop work assignments and schedule duty hours.
- Make daily rounds of the nursing service department to ensure all nursing personnel are performing their work assignments.
- Review complaints and grievances made or filed by department personnel.
- Ensure that disciplinary action is administered fairly.
- Participate in the interviewing and selection of residents for admission.
- Develop a written plan of care for each resident that identifies the problems/needs, indicated the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
- Prepare and plan the nursing service department’s budget and submit to the Administrator.
- Schedule, attend, and participate in nursing service meetings.
- Demonstrate a passion for caring as evidenced by interaction with co-workers, residents, families, and visitors.
- Perform all other duties, as assigned.
EDUCATION/EXPERIENCE/JOB TRAINING
- Must possess, as a minimum, a Nursing Degree from an accredited college or university.
- Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility. Must have, as a minimum (6) months experience in rehabilitative and restorative nursing practices.
Specific Requirements
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state.
- Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to deal tactfully when personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Must possess the ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
- Must be able to relate information concerning a resident’s condition.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as willingness to handle difficult residents.
- Must not pose a direct threat to the health or safety of other individuals in the workplace.

Responsibilities and duties include:
Making daily rounds, performing nursing assessments, evaluates changes in resident status and discusses status with DON when necessary, communicates with physicians on all matters related to resident care, assures accuracy of census reports, handles admissions and discharges, completes MDSs and trigger sheets, supervises feeding of residents, maintains proper documentation, communicates with resident family members and POAs, promotes and protects resident’s rights and assists with residents in making informed decisions. Assures all residents are treated with dignity and respect. Reviews and makes corrections on pharmacy and medical records, understands and oversees fire drills, elopement drills and other emergency preparedness activities. Functions as nursing supervisor in the absence of the DON/ADON.
Requirements:
Must be an RN in good standing in the state of Colorado, have obtained a bachelor’s degree from a four-year college or university or one-two years related experience and/or training or equivalent combination of education and experience. Must be CPR certified. Requires drug screen, criminal background check, and three positive references from previous employers.

MDS Assistant/Restorative Nurse

Liberty Heights is seeking a MDS Assistant/Restorative Nurse to coordinate or assist in the development and completion of the resident assessment process in accordance with the requirements of the Federal and state regulations as well as Company policy and procedures. This position reports to the MDS Nurse.
Essential Functions:
- Coordinates the development and completion of the resident assessment (MDS) in accordance with current Federal and state rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.
- Selects the correct Assessment reference date (ARD) and correct reasons for assessment that capture all resources utilized to ensure appropriate payment by the Medicare and/or Managed Care systems.
- Uses Senior Lifestyle systems (MC 100-day tracker, schedule notification, etc) to manage the MDS process.
- Assists in the development of a comprehensive resident assessment and care plan.
- Disseminates any new or updated materials involving the RAI process.
- Facilitates the involvement of appropriate health professionals needed to improve or maintain the resident's functional abilities at the highest practicable level.
- Creates an opportunity for family participation in the care planning process.
- Ensures the timely electronic submission of face validity of all Minimum Data Sets to the state data base.
- Reviews the state validation reports and ensures that appropriate follow-up action is taken.
- Serves on, participates in, and attends various other committees of the facility (e.g., Quality Assessment and Assurance) as required, and as directed by their supervisor.
- Communicates with the product champions to ensure that all new nursing associates are knowledgeable of the RAI process and receive the appropriate training as directed by their supervisor.
- Communicates with the Business Office Manager and Executive Director on a regular basis regarding default days and unassigned days.
- Understands, complies with and promotes all rules and regulations regarding residents' rights; promotes positive relationships with residents, visitors and regulators, to include presenting a professional appearance.
Non-Essential Functions: - Maintain confidentiality of all pertinent information.
- Attends all required training, in-service, and staff meetings.
- Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
- Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
- Adheres to all policies and procedures of Senior Lifestyle Corporation.
- Performs other duties as assigned.
Potential Exposure to Bloodborne Pathogens:
- This position may involve exposure to blood, body fluids, or tissue, requiring the use of personal protective equipment. Gloves should be worn for any contact with blood or body fluids/tissues. Gowns, masks, or eye covering may be needed if it is likely that soiling or splattering of blood may occur.
- To reduce the risk of exposure to blood, body fluid, or tissue adherence to Standard Precaution is necessary. In the event of accidental exposure to potentially infectious blood or body fluids/tissues, appropriate medical evaluation and follow-up is available as outlined in the community exposure control plan located in every department.
- It is the policy of this community to adopt work practices that will minimize health risks resulting from occupational exposure to blood and other potentially infected materials.
Qualifications/Skills/Educational Requirements:
- A current, active license to practice as a Registered Nurse or Licensed Practical Nurse in state of practice.
- Minimum two (2) years of clinical experience in a health care setting.
- Minimum of one (1) year of experience in a long term care setting. Prior experience as an MDS coordinator with demonstrated expertise.
